Directions:
- I-94 to Hwy F - Exit 295.
- Go South on F.
- Take F to Barstow and turn left.
- Take Barstow to the 5th set of stoplights, which is Wisconsin Avenue.
- Go to the next Road which is Park Street and turn right.
- Our parking lot is the second parking lot on the right side of Park Street. You will see signs in the parking lot indicating WFPC patient parking.
- Walk to the front of our building on Wisconsin Avenue and take the elevator to the second level.

When making an appointment, please describe the reason you wish to be seen. Doing so helps us schedule the right amount of time for your visit. If you are unable to keep your appointment, please call us as soon as possible so we may offer another patient your time. If you cannot keep your appointment and you fail to cancel it twice in a single year, you will be assessed a fee for each additional uncancelled appointment in that year.
